Print this form. You do not have to print these instructions or anything below.
Enter the quantity next to the appropriate shipping destination.
Multiply the quantity times the prices of the items you are ordering and enter that amount it the Total for the item.
Add all totals and enter the sum next to Amount Due.
Fill in the current date and your name and address below Ship To.
Make out your check or money order payable in U.S. funds to Michael Metras for the Amount Due. If you live in the EU, I can accept the equivalent in Euros. But I cannot accept any other foreign funds. Email me for our German account number to deposit your funds.

Shipping

Our printer prints your paper books on demand. That is, we print to your order. For that reason, it takes 3 to 5 business days to produce them before they are shipped by our printer. After that they are shipped by U.S. Media Mail to US destinations. The Post Offices says it takes 7 days to deliver.

For CDs and the video, we ship your order as soon as possible after we receive notice from PayPal of your payment.
